About Subacute Care

Subacute care is a level of care needed by a patient who does not require acute care provided in a typical hospital setting, but who requires more intensive skilled nursing care than is available in a skilled nursing facility.

Subacute patients are medically fragile and often require special services such as inhalation therapy, tracheotomy care, tube feeding, or complex wound management.

For patients who have had surgery, or suffer from an acute illness or injury, subacute care often acts as a bridge between the hospital, skilled nursing, and home.

Our Subacute Unit

We provide subacute care in a specialized 22-bed unit located within our main acute care hospital. Patients and their families have peace of mind knowing that all the benefits of a full size hospital are close at hand should additional care be needed.

Our team provides care with dignity and compassion. The size of our unit allows for increased patient interaction and individualized care plans. Our goal is to help each patient reach his or her optimal functional capacity.
